Admission for courses under Rajiv Gandhi University of Health Sciences.

Candidate seeking admission to the above courses under Rajiv Gandhi University of Health Sciences should fill in the application form, which can be downloaded from the Application Download section on this page, and the filled-in application is to be sent to “The secretary, Dr. M.V.Shetty Memorial Trust, A.B. Shetty Circle, Mangalore – 575 001, Karnataka along with a demand draft for Rs. 750/- (Rupees Five Hundred) drawn in favour of Dr. M.V. Shetty Memorial trust, Mangalore payable at Mangalore being the application and prospectus fee of Rupees 500/- and a registration Fee of Rupees 250/- ( Total of Rupees 750/- )

Attested copies of the following certificates should be sent along with the application form:

  • S.S.L.C. Certificate (10th)
  • Marks cards of the qualifying examination (10+2) or both 11th and 12th
  • Marks cards of Bachelor Degree Examinations for the admission into PG courses
  • Pass certificate of 10+2 issued by the examination conducting authority
  • Transfer certificate issued by the College/School
  • Conduct certificate issued by the Principal
  • Date of birth in Indian Almanac (A.D.)
  • Experience Certificate for admission to B.Sc Nursing (Post Certificate and M.Sc. Nursing Course).
  • Following Original Certificates should be submitted at the time of Admission with one set of attested copy:

  • S.S.L.C. (10th)
  • Marks cards of the qualifying exam i.e. 10+2 (both 11th and 12th Year)
  • Marks cards of all the years of the Degree Course for Pg admissions
  • Provisional Pass Certificate or Degree certificate issued by the University
  • Transfer certificate issued by the college
  • Migration certificate issued by the University/Board
  • Character/conduct certificate issued by the College/School last studied
  • Physical fitness certificate issued by the Government Medical Officer
  • Pass port size photograph – 8
  • For students from Nepal: Nepalese citizenship certificate and date of birth in Indian Almanac (A.D.)
  • For students from other country: Pass port and student visa and date of birth in Indian Almanac.
  • Eligibility certificate:Students who have completed their qualifying examinations outside Karnataka should apply for an eligibility certificate to the Rajiv Gandhi University of Health Sciences, Bangalore in a prescribed format.Application can be downloaded from(Download Section).The filled in application form along with the .D.D. of Rs. 1100/- drawn in favour of Dr. M.V. Shetty Memorial Trust, payable at Mangalore should be sent to the office of the Trust for sending the consolidated D.D. to the University for obtaining the eligibility certificate.
  • Migration Certificate: Candidates who have passed qualifying examinations outside the University to which the course now applied for is affiliated, should obtain and produce the Migration certificate from the University/Board under which the qualifying examination was passed.
  • Transfer Certificate: Candidates should produce the transfer certificate obtained from the School/College last studied.
  • Admission for Courses under Mangalore University

    Candidates seeking admission to these courses under the MANGALORE University, Mangalagangotri, Mangalore, should fill-in the application form downloaded from the Application Download section on this page, and the filled-in application is to be sent to “The secretary, Dr. M.V.Shetty Memorial Trust, A.B. Shetty Circle, Mangalore – 575 001, Karnataka along with a demand draft for Rs. 500/- (Rupees Five Hundred) drawn in favour of Dr. M.V. The filled in application form along with the prescribed fees should be submitted to the university.It is the duty of the candidate to obtain the Eligibility Certificate from the University and produce the same to the Institution before admission. However the Office of Dr. M.V.Shetty Memorial Trust helps the student in obtaining the Eligibility Certificate from the Mangalore University.
  • Migration Certificate:Candidates who have passed qualifying examinations outside the University to which the course now applied for is affiliated, should obtain and produce the Migration certificate from he University/Board under which the qualifying examination was passed.
  • Transfer Certificate: Candidates who have passed the qualifying examination should produce the transfer certificate obtained from the School/College last studied.
  • Special Instructions For B.A.S.L.P.

    Candidates who have passed 10+2 examinations under any University Board should produce the Marks cards of both 11th and 12th year. In case there is no examination conducted by the University/Board for the 11th year they should obtain a certificate from the college or university or board that the University/Board conducts no examination for the 11th std. and submit the certificate at the time of admission.
